Part-Time Home Care Physical Therapist

About the role

Provides patient care through direct treatment, evaluation, and follow-up, developing tailored care plans based on physician orders. Accurately documents patient progress and updates schedules for coordination. Communicates with healthcare team and discusses reimbursement policies with patients and families. Coordinates referrals and offers in-service, professional, and public education. Supports peer supervision and assists with staff training and research activities.

Qualifications

• Minimum bachelor’s degree in physical therapy and current state licensure
• Preferably over one year of therapeutic experience outside of home health
• Strong knowledge of physical therapy principles, evaluation, and treatment
• At least twelve hours of continuing education annually
• Willingness to travel and adapt to work conditions
