About the role

The Family Support Specialist at Health Connect America and our family of brands provides best practice, quality clinical services, ensuring all documentation is completed according to payer, state licensing agent, COA, and HCA standards and established guidelines. This position also assists with marketing for new referrals, communicates with customers, colleagues, and other individuals, develops treatment plans, implements therapeutic interventions, completes client documentation, attends and participates in treatment team meetings, links clients and families with specific services and resources, acts as a liaison with other professionals and agencies, actively recruits new referrals, and provides 24/7 on-call support.

Responsibilities

  • Provide best practice, quality clinical services, ensuring all documentation is completed per Payer’s, state licensing agent’s, COA, & HCA’s standards and established guidelines
  • Affirmatively assist with marketing for new referrals
  • Communicate with customers, colleagues, and other individuals to answer questions, disseminate, or explain information, and address complaints/concerns
  • Develop treatment plans with the input of the treatment team and the Clinical Supervisor
  • Provide individual, group, and family therapeutic interventions and skill building as outlined in the Treatment Plan
  • Implement Health Connect America approved therapeutic interventions as delineated on the treatment plan with clients and their families
  • Complete all client documentation within established time frames
  • Attend and actively participate in weekly Treatment Team meetings
  • Link the client and family with specific services and resources as identified in the treatment plan
  • Actively recruit new referrals to ensure that your caseload stays full
  • Provide 24/7 on-call support for clients as per program requirements
  • Ensure compliance with all state regulatory bodies & COA standards
  • Participate in and complete all assigned training and professional development activities, including E Learning courses

Requirements

A Bachelor's degree in a human services discipline such as Social Work, Psychology, Sociology, Counseling, or Criminal Justice, along with 1 year of experience working with children (including internships or volunteer experiences) is preferred, OR a BA/BS degree with 3 years of relevant experience.

Skills

Strong communication skills, ability to develop and implement treatment plans, and proficiency in therapeutic interventions are essential. Proficiency in Microsoft Office and familiarity with electronic health records (EHR) systems is also required.

Pay

Competitive salary based on experience and qualifications.

Schedule

Full-time schedule is typically 40 hours per week, with potential for flexible hours based on client needs.
