Part-Time Dialysis Social Worker

Role: Master Social Worker supporting dialysis patients and families within kidney care. Responsibilities: Conduct psychosocial assessments, develop and update care plans, provide counseling and crisis intervention, and navigate resources. Advocate for patients within an interdisciplinary team, coordinate with healthcare professionals to address social, emotional, and financial barriers, and support patient education and transition to dialysis care. Document in compliance with regulatory standards.
