Part-Time Dialysis Social Worker

Role: Master Social Worker supporting dialysis patients and families in kidney care. Responsibilities Conduct psychosocial assessments and develop care plans.Provide counseling, crisis intervention, and resource navigation.Advocate for patients within an interdisciplinary team including nurses, physicians, and dietitians.Coordinate to address social, emotional, and financial barriers to treatment adherence.Document in compliance with regulatory standards.Support patient education, adjustment to dialysis, and transitions of care. Qualifications & Skills Experience in social work, preferably in healthcare or dialysis settings.Excellent communication, counseling, and advocacy skills.Ability to collaborate effectively within a multidisciplinary team.Strong organizational skills for documentation and care planning.
