About the role

Serve as the initial clinical contact for patients through phone and in-person assessments.

Responsibilities

• Conduct symptom evaluations and prioritize care using evidence-based protocols to determine appropriate care levels.
• Coordinate same-day or follow-up appointments.
• Collaborate with physicians and care teams.
• Provide patient education, reinforce care plans, and support medication management.
• Facilitate test follow-up.
• Ensure efficient triage and support patient needs.
