Palliative Nurse Practitioner (APRN | Full Time)
Position Summary
The Advanced Practice Registered Nurse is responsible for working in collaboration with the patient, primary care physician, medical director, clinical nursing leadership and other palliative care team members in an active practice to deliver hospice and palliative care and related health care services to patients and families.
Primary Responsibilities
- Available as a consultant to patients, families and staff regarding end of life issues and to collaborate on symptom management, improve the patient and family experience of care, and improve the efficiency and effectiveness of resource use
- Provides direct patient care (or assists others to provide such care) to dying patients and their families in collaboration with the patient’s primary care physician, the hospice physician, hospice nursing staff and volunteers, and other members of the care team including Social Work and Chaplain Services
- Care is directed at meeting the physical, psychological, social and spiritual needs of the patient and family
- Prescribes or recommends drugs or other forms of treatment
- Records physical findings, and formulates plan and prognosis, based on patient's condition
- Consults with the Medical Director and palliative care physician(s) as needed
- Consults primary physician of services provided and collaborates with other physicians to prepare comprehensive patient care plan
- Maintains progress of patients and reports progress to Medical Director
- Has the ability to work collaboratively with hospice employees and volunteers as part of an IDG: Participates in Interdisciplinary Group Meetings
- Provides coverage of on-call during the extended hours and weekends as needed
- Provides training and continuing education to staff in collaboration with the education department
- Aids in the development of clinical practice guideline/standards/policies/procedures in support of quality care
- Identifies needs within different stakeholder organizations and recommends the development of systems within Navian Hawaii to meet those needs
- Completes required face-to-face visits as assigned
- Provides clinical assessment results to the hospice medical director after making face-to-face visits with patients, as assigned
- Participates in the recertification process and collaborates with the Interdisciplinary Team to promote coordination of patient care
- Maintains accurate, up-to-date records
Knowledge And Skills Requirements
- Demonstrates sound decision-making in the care of patients and families through use of the nursing process
- Work independently to plan and provide patient care
- Provide holistic nursing care to patients
- Identify psychosocial needs of caregivers and provide basic care to meet those needs
- Establish and maintain effective working relationships with staff, clients, family members, the public, and representatives of other agencies and organizations
- Communicate effectively with all levels of individuals
Experience And Education Requirements
- Valid Registered Nurse License in the State of Hawai’i
- Valid Advanced Practice Nurse License in the State of Hawai'i
- Valid DEA/State NED Certificate with full prescriptive authority
- Eligibility to obtain NPI Medicare enrollment
- 2 years of experience of direct patient care
- Hospice or Palliative experience preferred
- ELNEC training preferred

- Reliable transportation with valid registration and insurance
- TB clearance
- CPR BLS Certification
Working Conditions
- Indoors in patient homes or Hospice Hawai’i homes, nursing, and acute care facilities; may include outdoors in all types of weather
Work Location
This position is based in the field, requiring travel to patient homes, facilities, and offices throughout Oahu using a personal motor vehicle
Hours
Generally, Monday through Friday 8am-4:30pm
Position may require weekend, night, or holiday availability
Rotating weekend and night coverage
Salary
$108,000-$140,000 Annually
