Nurse Practitioner - Palliative (Full time 1.0/Days)
Serves as a member of the Choices Palliative Care team.
Serves as a member of the Choices Palliative Care team. Assists with the provision of palliative medicine in accordance with organizational, state, and federal regulations. Serves as a comprehensive direct care provider with demonstrated skilled proficiency in symptom management and advanced illness care. Provides critical thinking and clinical judgment to address the physical, emotional, psychological, and spiritual needs and reflect the goals of care of the patients and families receiving palliative medicine services. Provides clinical direction and expertise to the interdisciplinary team. Participates in goals of care discussions and development of plans of care that are clinically appropriate and that meet accepted palliative care and Choices Healthcare standards and guidelines. Collaborates and maintains communication with the patients’ attending providers, hospice and subspecialists for the coordination of palliative medicine services. Collaborates with Choices Palliative Medical Director and/or Lead Nurse Practitioner and additional members of the IDT when appropriate on matters of patient care. Monitors plans of care to assure consistency with Choices Healthcare philosophy, policies, procedures, and protocols. Provides comprehensive clinical and psychological evaluations of patients and their families in relation to their goals of care and serves as a patient advocate in the development of patient centered goals of care and documents visits and services according to Medicare and Choices Healthcare standards. Participates in interdisciplinary team meetings as scheduled. May be required to participate in adult hospice responsibilities as directed.
Qualifications
- Certified Nurse Practitioner license in the Commonwealth of Pennsylvania required
- Two Years Of NP Experience Required
- Hospice and/or Palliative Medicine experience preferred
- Prescriptive authority required
- Unrestricted DEA license required
- Certification in Hospice & Palliative Medicine required within three years from date of hire
