Compensation: $101,400.00 per year

Responsibilities

• Provide and document medically necessary services in accordance with physician/NP protocol
• Participate in the decision to recertify and discharge patients to/from hospice & palliative care
• Visit patients in home to assess recertification appropriateness for hospice and make treatment recommendations
• Write treatment orders and document as appropriate
• Work with the interdisciplinary care team to ensure that the medical needs of the client are met
• Develop the appropriate treatment plan that maximizes the health potential of the patient, including:
  • Ordering and interpretation of diagnostic tests
  • Identifying appropriate pharmacologic agents
  • Identifying appropriate non-pharmacologic interventions
  • Developing an education plan as appropriate
  • Interpretation of laboratory and radiology tests
  • Ordering of durable medical equipment
• Advanced care planning for the maximization of continuity of care
• Consult with collaborative physicians, consulting practitioners, primary care physician, and other specialist physicians
• Establish relationships with existing hospital-based hospice palliative care teams, partner organizations, medical directors, hospital physicians, community physicians, nurses, and other team members to build a culture of collaboration and clinically integrated care across the continuum
• In collaboration with PCP, refer patients to medical specialists and/or other healthcare services as medically indicated
• Assist in all facets of care coordination for referrals
• In association with the Marketing department, develop outreach programs for community agencies and healthcare settings
• Participate in outreach presentations and education to interested groups
• Provide disease management instruction and education to patients and their families
• Provide clinical guidance and education to facility staff related to patient care issues, assessments, and interventions
• Perform reasonable on-call services as needed
• Participate with care setting's interdisciplinary team as appropriate
• Prepare and maintain accurate patient records, charts, and documents to support sound medical practice and reimbursement for services provided
• Act as a clinical resource to coordinate complex cases for safe and appropriate transitions to other care settings
• Comply with applicable laws and regulations with respect to Delegation Protocol Agreements, prescriptive authority, and NP scope of practice
• Review KAQ policies and services with referred patients and/or family caregivers or authorized patient representative and obtain consent for medical care
• Work collaboratively with the Intake department in verification of coverage/payment and initiate reimbursement for services rendered
• Assist in the development and review of clinical practice guidelines/standards in support of quality care, and the preparation of associated policies and procedures
• Act with integrity, compassion, and customer focus to ensure quality care
• Adhere to employment policies as described in the employee handbook; maintain and respect confidentiality; adhere to all HIPAA regulations
• Participate in all required in-service education programs and meetings
• Demonstrate compliance with the dress code policy by appearing well-groomed, wearing ID badge while on duty, and maintaining a professional appearance
