Responsibilities

• Conducts comprehensive medical and social histories and performs physical examinations of healthy, acutely ill, chronically ill, or terminal patients.
• Treats common acute illnesses, chronic and stable health conditions through prescribing medications and other treatments.
• Orders and interprets lab tests and x-rays.
• Provides information, instruction, and counseling on health maintenance, health promotion, social problems, illness prevention, illness management, palliative care, and medication use.
• Refers patients to other health professionals as needed.
• Communicates with staff members, patients, and families to promote optimal patient outcomes.
• Completes documentation accurately and timely while maintaining state, federal, and agency regulations.
• Participates in quality improvement projects.
• Demonstrates a commitment to excellent customer service for internal and external customers.
• Performs all other duties as assigned.

Requirements

• Current unencumbered license for the state of Ohio to practice as a Certified Nurse Practitioner (CNP).
• Current Certificate of Prescriptive Authority or Externship Certificate to Prescribe.
• Two years of experience in the home health and/or hospice field.
• Proficiency with Windows, Microsoft Office (Word, Excel, PowerPoint), and the internet.
• Proficiency with electronic medical records (EMR) preferred.
• Must be able to read, write, speak, and understand the English language.
• Must possess a valid driver's license and acceptable driving record.
• Must be insurable under professional liability and crime coverage policies as specified by insurance carrier underwriting standards.

Working Conditions

• Sitting: Up to 8 hours/day
• Standing: Up to 8 hours/day

• Driving: Up to 4 hours/day
• Lifting, transferring, pushing, or pulling patients or equipment/supplies: Up to 50 pounds
• Work weekends, evenings, and holidays: Required as scheduled
• Subject to falls, burns from equipment, odors, and cuts: Workday
• Subject to patients with various disease processes: Occasional
• May be exposed to infectious waste, disease, conditions, etc., including exposure to the AIDS and Hepatitis B viruses: Workday
