Responsibilities

• Care and Relationship Management - 85%
  • Interprets symptoms and diagnoses conditions using knowledge of disease processes and epidemiology.
  • Obtains medical, social, and developmental histories.
  • Performs comprehensive and problem-focused history and physical examinations.
  • Prescribes, interprets, and correlates laboratory and diagnostic procedures.
  • Examines patients presenting with chief complaints or symptoms and evaluates their condition.
  • Provides treatments for injuries and refers patients to specialists (e.g., ophthalmologists, orthopedists, rheumatologists) as needed.
  • Interprets subjective and objective data to formulate differential diagnoses and treatment plans.
  • Offers advice on healthy habits and preventative actions to promote overall health.
  • Prescribes medications and provides instructions for administration.
  • Collaborates with physicians, physician assistants, nurse practitioners, and nurses.
  • Maintains comprehensive and accurate electronic medical records (EMR).
  • Cultivates a climate of trust and compassion for patients.
  • Stays updated on emerging healthcare models and best practices through conferences, seminars, CEUs, or CMEs.
  • Performs face-to-face patient encounters with complete Medicare-compliant documentation.
  • Assists in fostering effective working relationships with professional and administrative staff.
  • Performs additional tasks deemed necessary for efficient care delivery.
• Operations Management - 10%
  • Assists in health-related program development and evaluation.
  • Helps develop, implement, and manage educational programs for the organization.
• Quality & Compliance Management - 5%
  • Assists in developing and updating policies and procedures to comply with regulatory requirements.
  • Maintains confidentiality and rights of all patients and their care information.
• Performs all other assigned duties.

Requirements

• Education
  • Current unencumbered Ohio license to practice as a Certified Nurse Practitioner or Physician Assistant.
  • Narcotics and controlled substances licenses, permit numbers, and provider numbers for Medicare and Ohio Medicaid.
• Experience
  • Two years of experience as an advanced practitioner.
  • Experience in geriatrics preferred.
  • Knowledge of evaluation and management coding.
  • Experience in home health, hospice, physician office, or long-term care settings preferred.
  • Proficiency with Windows, Microsoft Office (Word, Excel, PowerPoint), and the internet.
• Other Requirements
  • Must read, write, speak, and understand English.
  • Valid driver’s license and acceptable driving record.
  • Must be insurable under professional liability and crime coverage policies.
