About the role

Palliative Care APRN wanted to join a diverse interdisciplinary team (IDT) that includes physicians, chaplaincy, social work, RN, and pharmacy support. This diverse practice can be tailored to your clinical interests, including:

  • Inpatient palliative medicine consulting service
  • Outpatient palliative clinic
  • 16-bed inpatient hospice unit
  • Home-based hospice visits

Shared call schedule conducive to quality work-life balance.

Responsibilities

  • Reviews clinical information and completes new consultations and follow-ups in a timely manner, working with the interdisciplinary team (physicians, social workers, and chaplains).
  • Acts as an integral part of the treatment team for patients with cancer-related illness and life-limiting chronic illnesses to assist in determining goals of care.
  • Manages symptoms for cancer-related patients via various routes in collaboration with the palliative care physician.
  • Provides extensive education regarding chronic diseases and their impact on quality of life.
  • Communicates with the treatment team, including other providers, on changes to the plan of care.
  • On-call coverage required, including cross-coverage for palliative care clinic, hospice inpatient, and home care teams.

Qualifications

  • Demonstrated skills and knowledge as a Nurse Practitioner in area of expertise.
  • One (1) year Nurse Practitioner experience preferred.
  • One (1) year hospice or palliative nursing experience preferred.
  • Excellent observation, problem-solving, mathematical, and nursing skills.
  • Knowledge of community, professional, and educational resources.
  • Excellent written and verbal communication skills among professionals.
