PACT Case Manager - Eyerly Ball

Responsibilities

• Conducting assessments
• Developing and implementing person-centered treatment plans
• Cooking and implementing treatment plans
• Coordinating medical and behavioral health services
• Providing crisis intervention including being available for rotating on call shifts for evenings/weekends
• Able to receive medication management certificate within first 3 months of employment
• Assisting with medication support
• Connecting clients to community resources and advocating for client needs

Qualifications

• Education: Bachelor’s Degree in Human services or related field Required
• Experience: One Year Of Mental Health Experience Required
• Licenses/Certifications: Valid driver’s license when driving any vehicle for work-related reasons. Required to obtain medication management cert within 3 months of hire. Ability to be CPR/First Aid trained.
• Knowledge/Skills/Abilities: Ability to speak, read and write fluent English. Ability to read and comprehend common terminology of the mental health field. Ability to enter documentation in an EHR and use Microsoft Office. Ability to effectively present information and respond to questions from clients, medical professionals, management, staff and the public.

Pay

Salary commensurate with experience.

Schedule

Monday-Friday 8-4:30. Occasional evenings and weekends.
