Job Summary
Support physicians, nurse practitioners, physician assistants, and/or nurses by taking vitals, assisting in medical examinations and other specific procedures and providing translation services.
Essential Job Functions
- Assists participants and supervises with activities of daily living (bathing, dressing, grooming, hair care, mouth care, eating, hand and foot nail care and toileting) as assigned or needed.
- Creates and participates in group(s) and one-on-one activities in accordance with daily activity calendar and as requested by the Recreational Coordinator.
- Monitor and assist participants with meals at feeding table.
- Accompanies enrollees to medical and other appointments.
- Performs and assists with laundry, including clothes for participants.
- Assists the drivers with the loading and unloading of participants from vehicles.
- Demonstrates dependability through consistent compliance with scheduled work hours.
- Participates in the activities of the interdisciplinary team as needed and carries out the plan of care as directed.
- Note changes in participant's physical or emotional health and communicate this information to the appropriate staff person.
- Maintains participants' medical record and fulfills VCC's PACE charting and reporting requirements.
- Assists the transportation department with participants' take-home supplies.
- Participates in and supports Quality Improvement initiatives.
- Follows all VCC's PACE Policies and Procedures and OSHA safety guidelines.
- Maintains the confidentiality of all company procedures, results, and information about participants, clients or families.
- P Practices standard precautions and follows appropriate Infection Control procedures.
- Maintains a safe working environment.
- Follows VCC's PACE Safety Policies and Procedures.
- Perform other duties as directed.
Minimum Qualifications
- High school graduate or equivalent
- CPR Certification (including BLS Child, Infant, Adult)
Preferred Qualifications
- Minimum of one years of experience in working with the frail or elderly population
- Bilingual English / Armenian
Skills/Knowledge/Abilities
- Skilled in establishing and maintaining effective working relationships with participants, co-workers, and the public
- Basic knowledge in working with frail and elderly to support Activities of Daily Living
- Skilled in identifying and recommending problem resolution
- Ability to work independently with minimum supervision
- Ability to communicate clearly and effectively
- Ability to work effectively with cultural, economically, and educationally, diverse populations
