Outreach Wkr-DCF
Key Responsibilities
- Offers structured one-to-one strength-based and resiliency building activities to the identified child or youth.
- Serves in natural community-based settings.
- Teaches social skills, anger management, and other related skills to youth.
- Provides transportation as needed to address care coordination and treatment needs.
- Supports family in accessing crisis intervention services as needed.
- Closely collaborates with program clinical staff to ensure excellent coordination of care.
- Participates in multidisciplinary team meetings and works with all behavioral health providers involved with the child in order to support the goals of the service.
- Coaches and supports the youth in expressing him/herself in these planning meetings.
- Provides daily documentation of interactions with the identified individual served specific to the goal area identified in the treatment plan.
Requirements
- A high school diploma required.
- A bachelor’s degree in the human service field, or five years of related experience working with children and families required.
- Individuals with lived experience in supporting a family member with mental illness or disabilities, or with personal lived experience preferred.
- Strong verbal and written communication skills.
- Ability to create and implement strategies to facilitate achievement of agreed upon goals.
- A valid driver’s license and reliable vehicle for all working hours required.
- Willingness and ability to work with client populations from diverse cultural and socioeconomic backgrounds.
- Knowledge of community resources and how to help clients access them.
- Ability to provide community-based service.
