Outreach Specialist
What You'll Do
- Reach out to participants before scheduled appointments to confirm awareness and rescheduling.
- Conduct escalating outreach to clients who fail to report to mandatory appointments and services using telephone, email, mail, and home visit strategies.
- Identify and assist with mitigating barriers to program participation, including health, mental health, reasonable accommodations, housing, childcare, and domestic violence issues.
- Re-engage clients by discussing available services and potential benefits of participation in WeCARE.
- Interdepartmental Coordination:
- Communicate informal accommodations to appropriate staff.
- Case conference with WeCARE Clinical Assessment, Case Management, and Vocational Rehabilitation Service (VRS) staff, as necessary.
- Work closely with VRS timekeeping unit team to maximize interactions and engagements with all WeCARE.
- Record-keeping and Compliance:
- Post necessary case notes of outreach services.
- Upload outreach documentation into HRA and Fedcap management systems, including SEAMS and FedcapCARES.
- Submit all records of escalating outreach interventions into HRA records systems.
- Perform other related duties as assigned.

In 1935, Fedcap Rehabilitation Services was founded by three men in NYC who had sustained injuries in WWI and literally changed the world for people with physical disabilities. It now provides an array of services to youth and adults helping thousands each year graduate from high school, obtain vocational certification or a college degree, become work ready, obtain meaningful employment, and achieve long-term economic well-being.

Qualifications
- High School diploma required.
- Bachelor’s degree in social work or related field, such as psychology, vocational rehabilitation, or occupational rehabilitation, preferred.
- One (1) year of human services or related experience.
- Bilingual (English Spanish) preferred.
Pay
$21.16-$22.12/hr./$44k-$46k annually
