Outreach & Enrollment Specialist

About the Role

The Outreach and Enrollment Specialist plays a key role in increasing access to health insurance coverage and quality primary, dental, and behavioral health care for both established patients and community residents. This position provides outreach, education, and enrollment assistance to uninsured and underinsured individuals, while cultivating partnerships with local organizations to support community health initiatives. The Specialist upholds the organization’s mission to deliver compassionate, competent, patient-centered care and contributes to a trauma-informed environment.

Location: Abbottsford - Falls, 4700 Wissahickon Ave, Philadelphia, PA 19144.

Responsibilities

• Offer structured education on health coverage options and provide follow-up support to maintain ongoing coverage.
• Provide one-on-one enrollment assistance for uninsured children and adults in subsidized, low-cost, and free health insurance programs, including the Health Insurance Marketplace, Medicaid, and CHIP.
• Assist patients with completing applications, gathering necessary documentation, and navigating the enrollment and renewal process.
• Engage with Lead Patient Service Representative (PSR) to link with all uninsured patients upon registration for appointments.
• Review Lapse Detection Report Lists and conduct outreach with uninsured individuals or those pending loss of insurance.
• Attend community events and partner health fairs to promote Outreach & Enrollment services.
• Participate in PA State Pennie assister monthly meetings and education sessions.
• Track and report application outcomes, enrollment data, and patient engagement encounters in the Athena (EMR).
• Complete weekly reporting requirements to PA CHC.

Requirements

• Bachelor’s degree in a related field preferred, or equivalent relevant experience.
• At least one year of experience with Pennsylvania health insurance programs, public benefits, and community resources is a plus.
• Ability to travel locally within the service area using personal or public transportation (mileage reimbursed).
• Ability to maintain confidentiality and handle sensitive information with professionalism.
• Ability to pass required background screenings and credentialing verifications.
• Ability to complete and pass the PA State Pennie Assister Training and obtain Assister License certification; maintain annual re-certification.
• Required TB/PPD test.
• Current valid CPR Certification.
• Maintain current, valid, applicable position credentials.
• Reliable transportation to travel throughout the network and/or participate in events outside the network as needed.
• Willingness to work a minimum of 40 hours weekly (Hybrid).
