Outreach Engagement Specialist

About the Role

The Outreach Engagement Specialist within the Trauma Recovery Center is responsible for engaging individuals identified by hospitals, emergency personnel, families, faith communities, and other social service agencies as appropriate referrals for TRC services. This role requires a passion for working with survivors of violent crime/trauma, including sexual assault, domestic violence, physical assault, shootings, gang violence, vehicular assault, human trafficking, and family members of homicide victims.

The population served focuses on underserved and at-risk groups, such as persons who are unhoused, from refugee populations, of diverse ethnicity or origin, and those with chronic mental illness, substance use disorders, disabilities, or severe trauma-related symptoms.

Responsibilities

• Perform assertive community outreach at hospitals (including Emergency Departments), victims’ service organizations, homeless camps, shelters, detox centers, faith communities, police departments, and other locations to engage individuals appropriate for TRC services.
• Build and maintain professional relationships with local community groups, service organizations, hospitals, police, jails, and courts.
• Provide assertive community outreach and clinical case management as part of the TRC treatment team, consistent with treatment plans.
• Assist members in completing eligibility applications for financial entitlements.
• Provide outreach and support in the member’s home, office, or community-based settings, as needed.
• Accompany members to appointments and interviews with agencies for victims of crime (e.g., Police/Sheriff’s Department, Pima County Attorney’s Office) to file police reports or restraining orders.
• Follow up on all referrals to assess outcomes and provide additional services as required.
• Conduct initial screenings for Trauma Recovery Center service eligibility and make appropriate referrals.
• Coordinate intake and enrollment with CODAC staff, supporting individuals until enrollment is completed.
• Assess and address barriers to treatment engagement.
• Track and maintain written records of community contacts and outreach for potential engagement.
• Consult and cooperate with community systems to facilitate linkage, referral, crisis management, advocacy, and follow-up to attain treatment goals.
• Provide support or assistance to other departments to ensure service engagement for the Trauma Recovery Center.
• Assist in wellness and reengagement outreach via telephone, home visits, and other means.
• Transport prospective members to intake sites and local service organizations for medical, legal, law enforcement, and other needed services, as appropriate.
• Provide crisis intervention and de-escalation of members, as needed.
• Attend staff meetings and scheduled trainings.
• Perform other related duties in accordance with agency growth and changes.

Qualifications

Education and Experience:

• Master’s or Bachelor’s degree in a healthcare-related field with no experience or
• Master’s or Bachelor’s degree in a non-healthcare-related field with 3 months of relevant experience or
• Associate’s degree in a healthcare-related field with 6 months of related experience or
• Associate’s degree in a non-healthcare-related field with 1 year of related experience or
• Certified Medical Assistant with 1 year of related experience or
• Certified Peer Support Specialist with 1 year of related experience or
• High School diploma or GED with 2 years of related experience.

Preferred Qualifications:

• Two years of experience working with Medication Assisted Treatment (MAT).
• One year of experience working with individuals with substance use disorders.
• Emergency Medical Technician (EMT) certification.
• Commission Certified Care Manager (CCCM), Certified Family Support Partner, or Certified Peer Support Specialist certifications.

Additional Requirements:

• Valid Arizona Driver’s license, proof of current insurance, and willingness to use a personal vehicle (driver’s license is an essential requirement).
• Clean Motor Vehicle Record—no more than 2 moving violations or license suspension in the past 3 years.

Skills

• Bilingual (preferred).
• Extensive knowledge of community resources.
• Extensive knowledge of behavioral health challenges, including impacts of mental illness, domestic violence, and dynamics of addictive behavior.
• Familiarity with various ethnic and cultural groups.
• Intermediate to advanced computer skills using MS Office products (Word, Excel, Access) and data importing/exporting.
• Ability to communicate effectively both orally and in writing.
