Outreach Coordinator - Heart and Vascular Heart Acquisition
Summary of Position
This role is primarily responsible for coordinating all educational and community outreach efforts related to the Advanced Heart Failure (AHF) Program which includes Heart Transplant and implantable left ventricular assist device (LVAD). Works under the direct supervision of the Director Transplant services while collaborating closely with the Transplant Program Manager and LVAD Program Manager. Coordinates outreach visits, organizes educational opportunities and strengthens relationships with referring practices. Advocates and markets the AHF program to the community and Physician practices, promoting the program in the community Donation Awareness events and in conjunction with Donate Life and the Gift of Life Donor Program.
Minimum Qualifications
- Bachelor Degree or equivalent experience (4 years)
Preferred Qualifications
- RN License preferred
- Previous experience as an Outreach Coordinator in a transplant program preferred
