Outreach Community Health Worker- Bilingual (Spanish/English)

About the role

The Outreach Community Health Worker will primarily maintain a presence in the community with patients to promote, motivate, and improve healthy living. The CHW will assist patients in utilizing resources and accessing services to overcome barriers in order to meet their health goals.

Qualifications

• High School Diploma or equivalent

Essential Functions

• Meet patients in a clinic, hospital, ER, and where patients are located and conduct needs assessment using Social Determinants of Health Tool
• Schedule weekly follow-up calls and home visits to patients
• Help patients with social issues like homelessness, substance abuse, hunger, transportation, and many other social issues that are impacting the members' lives
• Absorb and organize patients' records, make follow-up appointments with a Primary Care Physicians or Specialists
• Help patients with tasks related to getting medical care and working towards health goals by helping to plan healthy meals or find time to exercise
• Aid patients in accessing resources, including quality of care, childcare services, food, housing, and transportation
• Maintain relationships with the Community Based Organization
• Able to travel and meet patients where they are; may include local Churches, homeless shelters, and members' residences

Pay

The pay range for this position if hired to work in New Jersey is $21.69 - $30.64 per hour.

Schedule

This outreach role requires full-time, day shift - 9-5 PM with required flexibility for outreach events.
