Responsibilities
- Deliver compassionate, high-quality care to patients across the lifespan
- Partner with providers and care teams to ensure seamless, coordinated treatment
- Support patients and families in understanding health needs and next steps
- Document care accurately using our EHR system
- Participate in care huddles and team-based quality improvement initiatives
- Serve as a key member of our Patient-Centered Health Home model
Requirements
- CNA Certification, active and in good standing with the State of North Carolina
- Graduate of an accredited program
- Current CPR/BLS certification
- Valid Driver’s License
- One (1) year of clinical experience in a primary care, community health, or outpatient setting
- Experience with underserved or rural populations
- Familiarity with team-based care or PCMH/PCHH models
Qualifications
CNA Certification, active and in good standing with the State of North Carolina

Valid Driver’s License
One (1) year of clinical experience in a primary care, community health, or outpatient setting
