Outpatient Pharmacist

About the role

The Outpatient Pharmacist provides comprehensive pharmaceutical care within a high-touch ambulatory retail pharmacy, supporting medication access, prescription fulfillment, and transitions of care for patients across the continuum.

Responsibilities

• Verifying and dispensing prescriptions
• Performing clinical medication reviews and therapy assessments
• Counseling patients on safe and effective medication use
• Resolving insurance, billing, and medication access issues
• Partnering closely with physicians, nurses, care coordinators, and bedside teams to support discharge planning and continuity of care
• Supervising pharmacy technicians and ensuring efficient daily operations
• Maintaining professional interactions with patients to facilitate successful patient experiences
• Educating and training students, residents, other caregivers, and/or patients
• Accountable for drug use policy adherence

Requirements

• PharmD or Bachelor’s of Science in Pharmacy from an accredited College of Pharmacy
• Current state licensure as a Registered Pharmacist (RPh) or eligible in current state within 90 days of hire
• Basic Life Support (BLS) Certification through the American Heart Association (AHA)
• Completion of a Board of Pharmacy approved course in the administration of immunizations within 90 days of hire for those administering immunizations

Qualifications

• Completion of a Post-Graduate Year 1 Residency (PGY1), fellowship or relevant post-graduate pharmacy experience
• Post-graduate year 2 residency (PGY2) or relevant experience preferred for Clinical Pharmacists hired in a Specialized Pharmacy practice area
• Board Certification
• Licensed Pharmacist with ambulatory/outpatient pharmacy experience

Skills

• Ability to perform the job while standing and walking for prolonged times
• Manual dexterity sufficient to perform computer and dispensing functions of the job
• Ability to lift up to 20 lbs
• Reach, stretch, stoop, climb steps and sit
• Normal or corrected vision
• Ability to clearly communicate verbally by phone or in person
