We are seeking a highly skilled and compassionate Licensed Physical Therapist (PT) to join our Mobile Outpatient team in Salt Lake City, Utah. Responsibilities
- Conduct comprehensive evaluations and develop individualized treatment plans for patients with various physical therapy needs
- Deliver high-quality, patient-centered care in a mobile outpatient setting, working closely with patients and families to achieve their goals
- Implement and supervise treatment plans, ensuring effective and efficient patient care
- Collaborate with multidisciplinary teams to develop and implement comprehensive care plans
- Communicate effectively with patients, families, and healthcare professionals to ensure seamless care coordination
- Maintain accurate and detailed records of patient treatment and progress
Requirements
- Current licensure as a Physical Therapist in the state of Utah
- Master's or Doctoral degree in Physical Therapy from an accredited program
- Experience working in a mobile outpatient setting preferred
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
- Familiarity with electronic medical records (EMRs) and practice management software
