Outpatient Home Health Physical Therapist

Full‑Time or Part‑Time Outpatient Physical Therapist position providing home health services within senior living communities in Easton, Norton, and Attleboro. The role focuses on optimizing function and independence for older adults through evidence‑based interventions.

Responsibilities

• Make outpatient visits to clients in designated senior living communities.
• Perform diagnostic tests and measurements, including joint range of motion, transfer status, stability, ambulation patterns, strength, endurance, balance testing, and safety assessments.
• Develop and implement individualized care plans with manual therapeutic exercises, gait training, balance, and other interventions.
• Continuously assess and revise the PT plan of care and participate in multidisciplinary care plan meetings with community staff.
• Educate and instruct clients, family members, or community staff on therapy services and activities to promote health, safety, and independent living.
• Accurately document observations, interventions, and evaluations using a touch‑pad tablet.

Qualifications

• Current Massachusetts Physical Therapist license.
• Outpatient experience is a plus but not required; new graduate PTs are encouraged to apply.

Pay

Base pay ranges from $48.00 to $54.00 per hour, depending on qualifications.
