Outpatient Counselor

About the role

The Outpatient Counselor position at Whole Health Treatment Center involves primary responsibility for the well-being and attainment of treatment goals of assigned patients. This includes assessment, treatment planning, referrals, and discharge planning. The role also entails the provision of group and individual counseling and maintaining current required documentation.

Responsibilities

• Assessment and treatment planning for assigned patients
• Referrals to other healthcare professionals as needed
• Discharge planning to ensure successful transition to community support systems
• Provision of group and individual counseling sessions
• Maintaining accurate and timely documentation of patient progress and interactions

Qualifications

• MUST be credentialed by the State of Louisiana, and have active status as one of the following: PLPC, LPC, LMSW, LCSW or LMFT
• Three years experience in Outpatient Counseling preferred
• Basic knowledge of computer skills - Microsoft Office (Word, Excel, Powerpoint) preferred
• Must be able to read, write, and speak English

Certifications

• BLS certification required (obtainable within 90 days of employment)
• CPI certification required (obtainable within 90 days of employment)
