Position Overview
Occupational Therapy staff members assist with ensuring the health and well-being of residents by providing comprehensive, quality therapy services. This position is responsible for administering skilled occupational therapy treatments while ensuring that the highest degree of quality care is maintained at all times.
About the Role
Elderwood at North Creek is an 82-bed skilled nursing and rehabilitation facility located in a scenic, woodland setting near the Hudson River in the North Country. The facility provides a full continuum of care including skilled nursing, long-term care, subacute rehabilitation, and specialized therapy services. Our interdisciplinary team delivers personalized care in a warm, community-focused environment. With a bright, spacious rehab gym and state-of-the-art equipment, we offer physical, occupational, and speech therapy for both short-term and long-term residents.
Responsibilities
- Evaluate and develop individualized treatment plans for residents
- Administer skilled occupational therapy treatments
- Monitor and document resident progress and outcomes
- Collaborate with physicians, nursing staff, and interdisciplinary team members
- Educate residents and families on therapeutic techniques and care plans
- Ensure compliance with all regulatory and facility standards
Qualifications
- Current New York State Occupational Therapist License (required)
- Strong clinical, communication, and documentation skills
- Ability to work collaboratively in a team-oriented environment
- Experience in skilled nursing or rehab settings preferred, but not required

Occupational Therapist (OT) Responsibilities
- Provide skilled occupational therapy evaluations and treatments in accordance with physician orders
- Develop and update individualized treatment plans and care goals
- Maintain timely and accurate clinical documentation
- Collaborate with interdisciplinary teams regarding resident progress and discharge planning
- Participate in resident care planning meetings
- Support resident and family education initiatives
- Assist with mentorship and clinical support activities, as needed
Occupational Therapist (OT) Qualifications
- Graduation from an approved school of occupational therapy required.
- State license to practice occupational therapy required.
- Two years’ experience in occupational therapy preferred.
- Interest in occupational therapy treatment programs for the geriatric and subacute populations utilizing a team approach.
- Ability to direct others necessary.
- Ability to communicate well verbally and in writing required.
- Current, valid CPR/BLS credential.
- Sense of responsibility and ability to work well with residents, families, and other staff members required.
- Ability to work with geriatric and chronically ill residents, as well as other professionals in a friendly and helpful manner necessary.
- Ability to perform tasks to established standards of excellence required.
