Opwdd Care Manager (Remote)

About the role

OPWDD Care Manager (Remote) position coordinating services for adults with intellectual/developmental disabilities, managing a caseload, developing Life Plans, conducting home visits, handling crises, and serving as the hub for medical, behavioral, and social services. Remote role with quarterly travel to NY.

Responsibilities

• Coordinate services for adults with I/DD.
• Manage a 35‑40 person caseload: build Life Plans, conduct home visits, handle crises, and keep families and providers aligned.
• Perform 40% documentation in the EHR.
• Act as the hub for medical, behavioral, and social services.
• Close loops and ensure paperwork is completed.

Requirements

• OPWDD experience.
• Prior NY I/DD experience.
• Strong de‑escalation skills.
• HIPAA knowledge.
• Microsoft Suite proficiency.

Qualifications

• Bachelor’s degree + 2 years of relevant experience,
• OR RN + 2 years experience,
• OR Master’s degree + 1 year experience,
• OR previously worked as an MSC before July 1 2018.

Pay

$50,000 – $75,000 per year.

Schedule

Full‑time.
