About the role

We are seeking an Optometrist to work full-time (5 days per week) in Fort Wayne, IN and surrounding areas, including Auburn, Warsaw, and Marion. The position offers competitive compensation with uncapped earning potential, a $10,000 sign-on bonus for full-time providers (prorated based on schedule), and a base salary opportunity for $175k+ for full-time (5 days/week) providers.

This role provides clinical autonomy, flexible scheduling with no evening or weekend hours, an established patient base with no quotas, and full administrative support. All equipment and supplies, including a laptop with a proprietary EMR system, are provided.

Responsibilities

  • Provide general optometry services to residents of nursing homes, assisted living facilities, and long-term care facilities within an assigned territory.
  • Diagnose and treat refractive disorders and ocular disorders caused by age-related illnesses.
  • Examine, diagnose, treat, and manage diseases, injuries, and disorders of the visual system, the eye, and associated structures.
  • Identify related systemic conditions affecting the eye.
  • Enter patient information and complete required clinical documentation in our EMR system.
  • Travel daily to skilled nursing facilities as part of a mobile treatment model.
  • Perform other duties as assigned.

Requirements

  • Doctorate of Optometry (O.D.).
  • State professional license.
  • Valid driver’s license.
  • TPA certified/Legend Drug Certificate.
  • Ability to work independently on a daily basis.
  • Excellent written, verbal, interpersonal, and organizational skills.
  • Ability to use email, the internet, and learn other healthcare-related software.

Pay

Opportunity for $175k+ base salary for a full-time (5 days/week) provider, prorated based on schedule. Currently offering a $10,000 sign-on bonus for full-time (5 days/week) providers, also prorated based on schedule.

Schedule

Full-time (5 days per week) with flexible scheduling and no evening or weekend hours required.
