OP Nurse Coordinator I - Presbyterian Hospital, Digestive Health Care

Location: Presbyterian Hospital with hybrid flexibility after training. Hours: Monday through Friday, daylight hours on a 4x10 schedule (one day off per week).

About the Role

The Outpatient Nurse Coordinator is a Registered Nurse and a member of the care delivery team. This role is responsible for the global care of the patient, accountable for multiple aspects of the patient care package even when direct patient contact is minimized. The Outpatient Nurse Coordinator uses independent judgment and communication with patients, families, physicians, and community resources to assist patients in attaining outpatient health. This role demonstrates a commitment to the community and the nursing profession.

Responsibilities

• Triages patient situations and coordinates care with physicians.
• Assists physicians with all aspects of patient care, including assessment, evaluation, and education.
• Provides global, holistic care for patients, ensuring continuity beyond office or procedural visits.
• Accountable for outpatient treatment follow-up to maintain patient health between visits.
• Serves as the primary nursing resource for a patient caseload, facilitating necessary external services (e.g., homecare, support services).
• Develops and maintains productive working relationships internally and externally, demonstrating accountability, enthusiasm, and commitment.

• May coordinate staff training and educational activities.
• Functions as a senior nursing resource, assisting in the professional development of new staff and ancillary support.
• Creates an environment of open dialogue, inquiry, and continuous development by seeking feedback and improving practice.
• Demonstrates knowledge of growth and development principles across the lifespan and assesses data to meet age-specific patient needs.

Requirements

• Minimum of 3 years of nursing experience.
• BSN preferred.
• Current licensure as a Registered Professional Nurse in the practicing state or a multistate license under the Nursing Licensure Compact (NLC).
• CPR certification based on AHA standards (didactic and skills demonstration component) within 30 days of hire.
• Basic Life Support (BLS) or Cardiopulmonary Resuscitation (CPR) certification.
• Act 34 clearance.
• UPMC-approved national certification preferred.
