One Care Plan Advocate

OCES is expanding to 11 more towns covering the south shore area, including Quincy, Braintree, Weymouth, and others. We support older adults and individuals with disabilities by providing vital information and coordination of services, promoting healthy, safe living to help consumers stay active and engaged in their lives and communities.

About the role

As a One Care Plan Advocate in our Senior Care Options (SCO) department, you will empower consumers to meet their needs for Long Term Services and Supports (LTSS) through assessment, person-centered care planning, advocacy, and participation in each consumer's Interdisciplinary Care Team (ICT). You will promote individualized care plans that are cost-effective and improve quality of life.

Responsibilities

• Assist in identifying a more appropriate LTSS Coordinator (LTSC) if, after initial assessment, it is determined that an enrollee has special needs outside the Advocate's/LTSC's expertise.
• Develop or assist in developing the LTSS component of an Individual Care Plan (ICP) and participate as a full member of the ICT.
• Facilitate the provision of appropriate community LTSS and resources, including Personal Care Attendant services.
• Identify community-based alternatives to long-term institutional care.
• Monitor the provision and effectiveness of community services according to the ICP, upon request from the ICT.
• Conduct initial and ongoing assessments of the health and functional status of OCP enrollees.
• Represent the needs of the OCP enrollee by identifying communication and support needs, advocating for the enrollee, and providing education on LTSS to the ICT and enrollee.
• Participate in assessing the appropriateness of facility-based LTSS, including accommodation, accessibility, and equipment needs, upon request from the ICT.
• Perform other duties as assigned.

Requirements

• Considerable knowledge of the principles and practices of social work, as well as the social, emotional, and physical processes of aging.
• Ability to maintain confidentiality and obtain sensitive information through interviews, drawing appropriate conclusions and making suitable recommendations.
• Ability to establish and maintain harmonious working relationships with consumers and staff.
• Ability to interpret, understand, and adhere to complex regulations, standards, and procedures.
• Knowledge and skill necessary to work successfully within an interdisciplinary team, with clear oral and written communication abilities.
