Responsibilities

• Ensure patients at University Urology receive quality and comprehensive coordinated care services.
• Promote timely access to appropriate care, decrease emergency room visits, and increase utilization of preventative care services.
• Represent the multidisciplinary team as an advocate, interpreter, educator, and counselor for oncology patients, their families, and the referring public.
• Create and promote adherence to treatment and survivorship care plans developed in coordination with the patient, family, and multidisciplinary team.
• Serve as the primary contact point, advocate, and informational resource for patients, care teams, family/caregiver(s), payers, and community resources.
• Provide care and guidance to cancer patients from screening to survivorship, demonstrating knowledge and coordination of care.
• Work with patients to plan and monitor care, addressing unmet health and social needs.
• Develop a care plan with the patient and family/caregiver(s), including both treatment and survivorship plans post-treatment.
• Monitor adherence to care plans, evaluate effectiveness, and facilitate changes as needed.
• Systematically assess, plan, implement, and evaluate the care of oncology patients.
• Provide education and information to patients and families to ensure seamless, continuous, and comprehensive care.
• Initiate appointments for chemotherapy education and coordinate completion prior to chemotherapy initiation.
• Document patient teaching, including family and others, based on assessed needs, and respond to requests for information regarding disease processes, expected side effects, and community resources.
• Support families through difficult decision-making periods and assist in care coordination for patients and families, providing emotional support.
• Cultivate and support primary care and specialty provider co-management with timely communication and integration of information into the care plan.
• Assist in identifying "high-risk" patients (chronically ill and those with special healthcare needs).
• Coordinate patient tests, procedures, appointments, and treatments across healthcare disciplines, helping patients overcome barriers to care (e.g., transportation, financial counseling).
• Refer patients for supportive care services, including nutritionists, social workers, counselors, and financial counselors.
• Assist eligible patients in coordinating care with clinical trials coordinators.
• Participate in multidisciplinary consultations and/or tumor conferences, serving as a liaison between patients and medical staff/services.
• Serve as a clinical liaison to the Institutional Clinical Trials office to ensure seamless patient integration into approved clinical trials protocols.
• Deliver intravesical agents and provide clinical oversight at designated infusion centers and ambulatory care settings.
• Track, identify, and navigate bladder cancer patients into appropriate clinical trials and therapeutic pathways.
• Maintain regular and prompt attendance, working scheduled hours and overtime as required.
• Complete all mandatory department, educational, and hospital requirements.
• Adhere to current Infection Control and Safety Standards.
• Perform related duties as assigned.

Requirements

• Associate's degree in nursing from an accredited institution and 1 year of professional nursing experience.
• RN - Registered Nurse - State Licensure and/or Compact State Licensure in accordance with the Nurse Licensure Compact (NLC) for Alabama (required upon hire).
• ACLS - Advanced Cardiovascular Life Support (within 180 days preferred).
• Certification to administer chemotherapy (within 180 days required).
• Comparable combination of education and experience may substitute for the above requirements.
