Key Responsibilities
- Identify high-risk oncology patients and provide proactive outreach and follow-up, including in-person and telephone communication.
- Collaborate with physicians and care teams to develop individualized care plans, coordinate follow-up appointments, scans, and diagnostic tests.
- Ensure timely communication and coordination between the patient, family, and care team, acting as a primary point of contact.
- Serve as a liaison between referring physicians and the oncology team to ensure streamlined transitions in care.
- Monitor treatment progress, compliance, and follow-up needs; support patients in understanding their treatment plans and care pathways.
- Coordinate medication refills, imaging, lab orders, and necessary authorizations in partnership with the provider and financial coordinator.
Patient Education & Support
- Provide anticipatory guidance and education for patients with chronic diseases or new diagnoses, emphasizing self-management and care ownership.
- Teach patients and families how to schedule scans, arrange transportation, and navigate the healthcare system.
- Integrate patient and family education throughout all phases of care, adapting to individual learning needs and cultural considerations.
Clinical Support & Communication
- Triage abnormal lab results and clinical messages for providers.
- Manage prescription refill requests and clinical inquiries.
- Support providers in addressing urgent patient issues and care coordination.
Program Development & Quality Improvement
- Participate in quality improvement initiatives to enhance patient outcomes and satisfaction.
- Contribute to accreditation efforts and the ongoing development of the cancer program through data collection, education, and process improvement.
- Maintain awareness of and adherence to regulatory requirements, hospital policies, and best practices in oncology nursing.
Qualifications
- Required Education: Bachelor of Science in Nursing (BSN)
- Licenses & Certifications: Registered Nurse (RN) – Current Maryland license or eligibility to obtain, Basic Life Support (BLS) – Current certification
- Preferred: Oncology Certified Nurse (OCN) certification strongly preferred

- Ability to manage multiple priorities, coordinate care across disciplines, and provide empathetic support
