Position Summary
Nurse Navigator facilitates all stages of the patient referral process. Works in direct coordination and cooperation with referring physicians, specialists, and self-referring patients. The Nurse Navigator is the resource/main contact for patients during the initial consultation phase.
Essential Functions and Responsibilities
- Facilitates all stages of patient referral process
- Maintains operational responsibility for navigation program and development
- Schedules all appointments with specialists properly
Requirements
- Licensed Registered Nurse with Bachelors Degree in Nursing
- Oncology Certified Nurse within 18 months of transfer/hire
