About the role
Thyme Care Inc., the management company to Thyme Care Medical PLLC, is seeking a Thyme Care Oncology Nurse Navigator. This role is for Thyme Care Medical PLLC and its patients.
Responsibilities
- Triage, support, and educate members and their care network via phone, email, and video communication.
- Conduct comprehensive clinical assessments and oversee member health.
- Facilitate end-of-life care discussions under the guidance of the Nurse Team Lead.
- Engage with members, addressing clinical issues and managing escalations.
- Collaborate with Nurse leaders and Medical Directors to align with clinical protocols and best practices.
- Establish trusting relationships with members and their care network, prioritizing empathy and active listening.
- Adhere to Care Team policies, procedures, and documentation standards.
- Support members throughout the oncology care continuum, from screening to survivorship or end-of-life care.
- Identify and address member needs, offering assistance with care coordination, symptom management, nutritional support, discharge planning, and provider referrals.
- Participate in case conferences to monitor member progress and collaborate on support plans.
- Foster strong partnerships with payers and providers to optimize care delivery and minimize readmissions.
- Collaborate with non-clinical Care Team members to address social determinants of health needs.
- Be available for urgent clinical escalations and provide clinical consult support as required.
Qualifications
- An active, unrestricted compact Registered Nurse (RN) license is required.
- A degree in Nursing (ADN or BSN) is required; a BSN is preferred.
- Compact + single state licensure in New York and/or Illinois is preferred.
- At least 5 years of nursing experience, including 3 years in solid tumor oncology nursing.
- Certifications in Oncology-related areas such as OCN, AOCN, AOCNS, or CCM are required or obtained within 2 years of hire.

Pay
The pay rate for this role is $41.54/hour.
Schedule
This role follows a fixed schedule of 8:30 AM to 5:00 PM EST, working four weekdays and Saturdays. The designated weekday off must be Monday, Thursday, or Friday.
