Role: Nurse Navigator RN
Responsibilities
- Guide patients through complex care pathways in a dynamic environment
- Coordinate appointments, referrals, and multidisciplinary services for seamless patient transitions
- Conduct clinical assessments and provide patient and family education
- Identify and address barriers to care to ensure optimal outcomes
- Document all interactions accurately in EMRs, maintaining safety and quality standards
- Collaborate with EMS and hospital partners to deliver compassionate, life-impacting care
- Enhance specialized clinical skills through ongoing collaboration and learning
Qualifications & Skills
- Registered Nurse (RN) license
- Strong clinical assessment and patient education skills
- Excellent coordination and communication abilities
- Ability to work effectively in a fast-paced, team-oriented setting
- Commitment to safety, quality, and compassionate care
