Department: Hematology Oncology
Schedule: Full Time, 40 Hours

Location: 12 High Street, Lewiston, Maine 04240
Central Maine Healthcare (CMH), a member of Prime Healthcare Foundation, is an integrated healthcare delivery system serving approximately 400,000 residents across central, western, and midcoast Maine through a network of more than 40 primary and specialty care locations.

Responsibilities

• Coordinating service and identified patients through a complete health system by assisting with access issues, identifying resources, providing educational material, and developing relationships with service providers and community.
• Carefully coordinating the care in a timely and efficient manner, identifying and eliminating barriers to care.
• Consistently serving as a care coordinator throughout the continuum of care and assessing the physical, psychological, and social needs of the patient, enhancing patient outcomes, increasing satisfaction, and reducing cost of care.
• Engaging in community education and screening, as well as monitoring community outreach and promoting survivorship.

Qualifications

• Current and valid state RN License.
• Current BLS certificate upon hire and maintain current.
• Bachelor of Science in Nursing (BSN) preferred.
• Minimum of two (2) years of nursing experience.
