Oncology Medication Mgmt Pharmacist-Ambulatory Pharmacy Practice-FT Variable Shift
About the role
The Medication Management Pharmacist is responsible for providing patient-centered clinical services, including comprehensive medication therapy review, medication information/education, and adherence to evidence-based medicine and national guidelines.
Responsibilities
- Review patient medical records and perform comprehensive medication therapy reviews
- Provide medication information/education to providers and patients
- Manage referred patients under a collaborative practice agreement
- Complete comprehensive medication reviews for high-risk patients and those at high risk for rehospitalization
- Perform prior authorization reviews and deferred authorization requests
- Serve as a subject matter expert and supervise pharmacy technicians
- Precept pharmacy residents, students, technicians, or interns as assigned
Requirements
- Registered Pharmacist in good standing licensed by the state of California
- BLS or ACLS upon hire
- Completion of a Doctor of Pharmacy degree from an accredited School of Pharmacy
- Completion of a PGY1 pharmacy practice residency and a PGY2 Ambulatory Care Pharmacy Residency or 2+ years experience as a clinical pharmacist in an ambulatory setting
Qualifications
- Training and knowledge of specialty areas (oncology, hepatology/gastroenterology, neurology, dermatology)
- Experience working within a multidisciplinary team as part of a Comprehensive Medication Management model
Skills
- Specialty/retail pharmacy, ambulatory clinic pharmacy, and/or PBM experience
