Medication Mgmt Pharmacist-Ambulatory Pharmacy Practice-FT Variable Shift

About the role

The Medication Management Pharmacist provides patient-centered clinical services, including comprehensive medication therapy review, medication information/education, and adherence support. They also supervise pharmacy technicians and precept pharmacy residents, students, technicians, or interns.

Responsibilities

  • Review patient medical records and perform comprehensive medication therapy reviews
  • Provide medication information/education to providers and patients
  • Apply evidence-based medicine and national guidelines to optimize medication-related health outcomes
  • Manage referred patients under a collaborative practice agreement
  • Complete comprehensive medication reviews for high-risk patients and those at high risk for rehospitalization
  • Perform prior authorization reviews and deferred authorization requests
  • Supervise pharmacy technicians and ensure completion of medication histories
  • Precept pharmacy residents, students, technicians, or interns as assigned

Requirements

  • Registered Pharmacist in good standing licensed by the state of California
  • BLS or ACLS upon hire
  • Completion of a Doctor of Pharmacy degree from an accredited School of Pharmacy
  • Completion of ASHP-accredited PGY1 pharmacy practice residency and a PGY2 Ambulatory Care Pharmacy Residency or 2+ years experience as a clinical pharmacist in an ambulatory setting

Qualifications

  • Training and knowledge of specialty areas (oncology, hepatology/gastroenterology, neurology, dermatology)
  • Experience working within a multidisciplinary team as part of a Comprehensive Medication Management model

Skills

  • Specialty/retail pharmacy, ambulatory clinic pharmacy, and/or PBM experience
