About the role

The Oncology and Infusion Staff II Pharmacist provides pharmaceutical care to patients in all areas where pharmaceutical services for Infusion/Adult Hematology/Adult Oncology are provided within the UCSF Health System. They may also participate in pharmaceutical care in other sectors of the Department of Pharmaceutical Services as needed.

The role involves performing a wide range of pharmaceutical care functions to ensure drugs prescribed for and used by patients are safe and effective. This includes obtaining medication histories, reviewing and assessing medication profiles, providing drug information to patients and healthcare providers, collaborating with clinicians on therapeutic plans, ensuring medication access, educating patients on proper medication use, and ensuring patients understand cautions, adverse reactions, and interactions related to their therapy.

The Oncology and Infusion Staff II Pharmacist will be part of an interprofessional team consisting of physicians, advanced practice providers, nurses, and other professionals. They will be based in the ambulatory or inpatient setting, managing injectable, oral, and/or infusion medications for patients with hematologic and oncologic malignancies. Depending on the assigned shift and site, responsibilities may include overseeing pharmacy technicians for sterile compounding or facilitating insurance authorizations for medications. The role may also involve oversight of clinical activities, delivery of services, and the Patient Management Program.

Staff are expected to participate in department initiatives to improve quality and safety. This position requires flexibility to orient and work at all UCSF Health locations depending on operational needs.

Responsibilities

• Obtain medication histories and review/assess medication profiles.
• Provide drug information to patients and healthcare providers.
• Collaborate with clinicians on drugs of choice and therapeutic plans.
• Ensure access to medications and educate patients on proper use.
• Ensure patients understand cautions, adverse reactions, and interactions related to their therapy.
• Manage injectable, oral, and/or infusion medications for patients with hematologic and oncologic malignancies.
• Oversee pharmacy technicians for sterile compounding, as applicable.
• Facilitate insurance authorizations for medications.
• Oversee clinical activities, delivery of services, and the Patient Management Program, as applicable.
• Participate in department initiatives to improve quality and safety.
• Work flexibly across all UCSF Health locations based on operational needs.

Requirements

• Doctor of Pharmacy degree (PharmD) or equivalent degree as determined by the National Board of Pharmacy.
• Current license in good standing to practice pharmacy in the State of California.
• Current certification for Basic Life Support (BLS) from the American Heart Association.

Qualifications

• Preferred: Completion of an ASHP-accredited PGY1 Pharmacy Residency and/or Fellowship, or at least 3 years of Oncology/Infusion experience.
• Preferred: Completion of an ASHP-accredited PGY2 Oncology Pharmacy Residency and/or Fellowship, or at least 3 years of Oncology/Infusion experience.
• Preferred: Advanced board certification (e.g., BCOP, BCPPS, or BCPS).
• Preferred: Operational experience in Oncology and/or Infusion Pharmacy.
• Preferred: Experience in sterile compounding of hazardous and non-hazardous drugs.
