Responsibilities

  • Assume responsibility for evening, night, weekend, and holiday on-call duty as scheduled, consistent with Agency policies and procedures.
  • Manage primary responsibility for total region census during assigned shift, including assessing, planning, implementing, and evaluating phases of the nursing process.
  • Maintain regular communication with Nurse Supervisors, attending physicians, and case managers concerning patient/family care needs and response to interventions.
  • Respond to medical or emotional crises and make home visits to provide direct care as appropriate.
  • Review referrals and initiate contact with clients and families.
  • Conduct complete nursing assessments based on referral data, client/family interviews, and clinical observation of patient health status.
  • Demonstrate clinical expertise in symptom management.
  • Develop and modify a plan of care in collaboration with clients and all disciplines, documenting need for services, discharge planning, goals, and resource utilization.
  • Demonstrate understanding of payer information to maximize care planning.
  • Initiate and maintain client records utilizing the Agency’s documentation standards.
  • Make follow-up visits to provide skilled care, assess disease progress, evaluate treatment response, and coordinate care with providers.
  • Teach patients and reinforce client/caregiver education, including signs and symptoms, to promote health and prevent disease.
  • Refer, collaborate, and coordinate all client care services, consulting with physicians regarding client needs and plan of care.
  • Direct LPNs, Home Care Aides, and supervise their plan of care.
  • Refer clients/families to other community resources as needed.
  • Attend and participate in in-service education programs.
  • Seek consultation and guidance from supervisors.
  • Complete reporting requirements on a timely basis.
  • Actively participate in the H3W improvement process to identify service line needs and development.
  • Perform other duties as assigned.
  • Provide skilled nursing care to clients in their homes or community health settings, ensuring delivery of outcome-based, high-quality, cost-effective care.

Requirements

  • Graduate of an Associate’s or diploma program in nursing (required). Graduate of an NLN-approved BSN program (preferred).
  • One year of nursing experience or one year of nursing in a specialty (required); medical/surgical or home care experience (preferred).
  • Registered Nurse with a license to practice in the state of Connecticut (required).
  • Computer skills.
