Registered Nurse On-Call - Home Health Visits

We are seeking an On-Call Registered Nurse to provide home health visits to clients throughout Baltimore County and Baltimore City. This role focuses on after-hours phone triage and client visits.

About the role

• After-hours phone triage and home visits
• Home Health experience strongly preferred
• Weekday schedule: Monday–Thursday, 5:00 PM–8:30 AM

Responsibilities

• Follow a designated care plan in accordance with patient’s needs
• Make home visits to clients in designated geographic territories
• Perform assigned duties, including administration of medication, wound care, treatments, and procedures
• Monitor clients’ conditions; report changes to Clinical or Client Services Manager
• Follow up with, execute, and properly document doctors’ orders
• Perform client assessments as necessary
• Case management and coordination
• Accurately document observations, interventions, and evaluations pertaining to client care using a state-of-the-art touchpad tablet

Requirements

• Current Maryland Registered Nurse license
• Minimum of one year of recent, verifiable clinical (medical/surgical) experience
• Prior home care experience strongly preferred
• Graduation from an accredited and approved nursing program
• Ability to work independently and manage time effectively
• Strong interpersonal and computer skills; prior EMR experience preferred
• Ability to travel to cases as assigned

Pay

Base pay: $55.00–$65.00 per point, depending on qualifications.
