About the role

The role involves providing on-call patient visits during after-hours for urgent physical and psycho-social issues, coordinating with the Call Center Triage and On Call Manager.

Responsibilities

• Responding to visit requests via cell phone
• Communicating with the on-call team and hospice physicians to address urgent needs
• Implementing and educating patients and caregivers on care plans, medication management, safety, and symptom control
• Attending patient deaths to support families
• Reporting visit details and care changes
• Ensuring adherence to NYS standards for Registered Nurses

Qualifications

• Graduation from an accredited Nursing program
• At least two years of experience in Medical-Surgical, Community Health, Hospice, or Oncology nursing
• NYS RN licensure

Skills

• Proficiency in Word, Excel, and Windows applications
• Certification as a CHPN is expected within one year of hire
