Job Summary

This is a full-time professional position providing after hours nursing coverage for patients on assigned weekends, holidays, and week nights to provide after hours nursing support to patients and families who have questions, concerns, or emergencies. This position collaborates with the manager on call and the weekend on call triage nurse.

Responsibilities

  • Plan, provide, supervise and document safe, competent, and compassionate hospice nursing care for patients in accordance with physician orders and established policies and procedures.
  • Provide after hours clinical and emotional support for patients and families in crisis.
  • Aid the triage nurse in determining and completing appropriate coverage of after-hours phone calls from patients and families.
  • Ensure current knowledge of nursing practices.
  • Provide and document appropriate and relevant end-of-life information and education to patients and their families in compliance with current Centers for Medicare and Medicaid Services (CMS) Conditions of Participation (CoPs).
  • Maintain required competencies, identify learning needs, and seek appropriate assistance or educational offerings for professional development.
  • Manage time, people, and technology effectively.
  • Monitor and manage the appropriate use of patients' medical equipment, supplies, and medications.
  • Collaborate with other Interdisciplinary Group (IDG) members to determine the patient's level of care and coordinate the appropriate utilization of contract services.
  • Participate actively in the orientation and training of new employees of all disciplines.
  • Work collaboratively with IDG and other Transitions LifeCare (TL) departments in achieving patient care goals.
  • Delegate tasks and supervise the activities of LPNs and Hospice Aides, as needed.
  • Perform other duties as assigned by supervisor.

Requirements

  • Registered Nurse with current NC license; BSN preferred.
  • Minimum 3 years recent clinical experience. Hospice and/or home health or nursing home experience or equivalent preferred.
  • CHPN Certification preferred.
  • Knowledgeable about reimbursement sources, Medicare regulations, licensing laws, and accreditation standards.
  • Competent computer skills.
  • Current CPR certification.
  • Valid North Carolina Driver’s License, automobile insurance coverage, and reliable transportation.
  • Able to travel to, enter, and function in the homes/facility of any patient/family in the service area to assess and provide care to all patients.
  • Able to lift and/or carry items up to 50 pounds.
  • Sensory abilities to complete physical assessments, communicate with patient/family care providers, and physician, and to complete the reporting and follow-up documentation responsibilities of the job.
  • Available to work on or around holidays as scheduled and potentially on a rotating basis.
  • Attend at least 4 team meetings per year.
