Position Summary

Lakes Region VNA and Hospice is seeking a dedicated and compassionate Registered Nurse (RN) to join our team. This part-time, on-call role supports the after-hour needs of our Hospice and Homecare patients. The schedule is Sunday through Thursday, 4 pm – 8 pm and Monday through Thursday, 5 am – 8 am.

Responsibilities

• Triage patient calls escalated from the on-call triage service and provide appropriate guidance and support.
• Make visits to patients’ homes to assess and address their needs.
• Collaborate with the interdisciplinary team to ensure comprehensive patient care.
• Document patient interactions and care accurately and timely.
• Provide emotional support and education to patients and their families.
• Provide end-of-life support for patients and families.
• Adhere to infection control protocols and safety guidelines.
• Participate in team meetings and ongoing training sessions.

Qualifications

• Current and valid RN license in New Hampshire.
• Bachelor’s degree in nursing (BSN) preferred.
• Previous experience in a home health care and hospice setting is preferred.
• Reliable transportation and a valid driver’s license.
