About the Role

Evolve Therapy Services is seeking compassionate Certified Occupational Therapy Assistants for our partner’s in-house Skilled Nursing Facility in Tupper Lake, NY. Full-time (with sign-on bonus), part-time, and PRN positions are available.

The purpose of this role is to plan, organize, develop, and direct Occupational Therapy Services in accordance with federal, state, and local standards, guidelines, and regulations. This ensures the highest degree of quality resident care is maintained at all times.

Responsibilities

• Conduct regular screenings of residents to determine the best intervention or treatment based on scope of practice.
• Perform treatment plans for residents, following short- and long-term goals and methods outlined in the established plan of care by the supervising therapist.
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members.
• Establish individual resident therapy schedules in cooperation with other team members.
• Conduct therapy according to the established treatment plan, ensuring proper documentation and billing of occupational therapy services.
• Comply with all CMS documentation standards of practice, including point-of-care documentation as indicated.
• Perform all other duties as assigned and as required to effectively treat patients.

Requirements

• Associate's degree in Occupational Therapy.
• Active OTA licensure in the state(s) of practice.
• Ongoing continuing education and professional development to maintain current licensure and certification.
