About the role

Evolve Therapy Services is hiring compassionate Occupational Therapists for an in-house Skilled Nursing Facility in Tupper Lake, NY. Full-time (with sign-on bonus), part-time, and PRN positions are available.

The purpose of this role is to plan, organize, develop, and direct Occupational Therapy Services in accordance with current applicable federal, state, and local standards, guidelines, and regulations, ensuring the highest degree of quality resident care.

Responsibilities

• Conduct regular screening of residents to determine the best intervention or treatment for each individual patient.
• Evaluate residents to inform and develop treatment plans; establish short- and long-term goals and methods to achieve goals.
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members.
• Establish individual resident therapy schedules in cooperation with other team members.
• Conduct therapy according to established treatment plans and ensure proper documentation and billing of occupational therapy services.
• Oversee Certified Occupational Therapy Assistants (COTA) as needed, ensuring all documentation and treatments are carried out with clinical excellence and proper reimbursement practices are followed.
• Perform all other duties as assigned and as required to effectively treat patients.

Requirements

• Degree in Occupational Therapy.
• Active Occupational Therapy license in the practicing state(s).
• Ongoing continuing education and professional development to maintain current licensure and certification.
