Occupational Therapy Assistant/COTA (Full-time)

HealthMark Rehabilitation is an in‑house therapy company serving Skilled Nursing Facility patients and residents of HMG facilities. This full‑time Certified Occupational Therapy Assistant (COTA) position provides therapy to residents and short‑term patients at the Stonegate facility in Fort Worth, Texas (4201 Stonegate Blvd, Fort Worth, TX 76109).

Responsibilities

• Provide therapy services in accordance with physician orders and accepted professional practices.
• Develop an overall therapy plan of care that includes treatment justification, planned duration and frequency, short‑ and long‑term functional goals, and discharge planning, following standards of practice and established guidelines.
• Perform skilled treatments according to the established plan of care.
• Document observations, patient responses to skilled interventions, progress, and notable changes in condition within required timeframes.
• Complete all documentation promptly and in compliance with state and federal regulations, policies, and guidelines.
• Submit billing on the date services are provided, adhering to all applicable regulations, policies, and guidelines.
• Assist with maintaining rehab gym equipment and treatment areas in a safe manner, employing proper infection‑prevention methods.

Requirements

• Hold a Texas license/certification for the appropriate discipline (COTA).
• Strong oral and written communication skills.
• Organizational and analytical abilities.
• Ability to function effectively within multi‑faceted systems and interact with all staff and customers.
• Demonstrated time‑management and organizational skills.
• Independent and effective clinical judgment, problem‑solving, and decision‑making skills.
• Physical ability to meet the demands of the role.
• Experience in a geriatric rehabilitation setting preferred.
• Successful completion of the hiring process and background check.
