Occupational Therapist (Sign-On Bonus)

About the role

Seeking a licensed Occupational Therapist to provide personalized, in-home and community-based care to older adults, supporting their mobility, independence, and quality of life through evidence-based treatments.

Responsibilities

• Conduct evaluations and develop individualized care plans
• Deliver therapy sessions in home and community settings
• Document care in electronic medical records (EMR)
• Collaborate with an interdisciplinary team including physical therapists, speech-language pathologists, and fitness specialists
• Incorporate telehealth and virtual care to enhance flexibility and patient access

Requirements

• Valid Occupational Therapy license
• Degree from an accredited Occupational Therapy program
• Computer proficiency
• Current CPR certification

Physical demands

• Assisting patients with mobility needs
• Lifting and repositioning patients
• Traveling to patient locations
