Responsibilities

• Provide one-on-one care to older adults in private residences and senior living communities.
• Perform evaluations.
• Develop personalized care plans.
• Deliver therapy sessions supporting wellness and functional independence.
• Document clinical work in electronic medical records per regulatory standards.
• Collaborate with an interdisciplinary team including PTs, SLPs, and fitness specialists.
• Educate patients, caregivers, and staff on safety and aging in place.
• Incorporate telehealth when appropriate for increased flexibility and care continuity.

Qualifications

• Valid OT license.
• Degree from an accredited program.
• Basic computer skills.
• CPR certification.

Requirements

• Physical demands involve moderate effort, patient transfers, repositioning, and driving to patient locations.
