Occupational Therapist PRN - Home Health

About the role

We are seeking an Occupational Therapist for home health services to assess and evaluate patients' functional status, activities of daily living, fine motor skills, and adaptive equipment needs.

Responsibilities

• Providing services within the scope of practice, following the plan of care, and using evidence-based techniques in collaboration with the healthcare team
• Evaluating patient functional status, documenting required CMS information, and consulting with physicians for therapy planning
• Monitoring and reporting patient responses and condition changes to supervisors and physicians
• Conducting assessments to determine levels of function using diagnostic and prognostic tests

Qualifications

• Current state licensure in Occupational Therapy
• CPR certification
• Valid driver's license, vehicle insurance, or access to transportation
• Ability to work with diverse populations regardless of background or condition

• Ability to manage multiple tasks independently
• Strong communication, organizational, and writing skills
