Occupational Therapist

  • Duties and Responsibilities:
    • Provide skilled occupational therapy services/interventions in accordance with Physician orders
    • Assess patient needs, plan, evaluate, and modify care to meet goals of Occupational Therapy Interventions
    • Collaborate with all disciplines to plan and evaluate team goals for each patient
    • Instruct patient/family/caregivers on patient's total Occupational Therapy program
    • Advise, consult, and instruct members of the multi-disciplinary team in specific occupational therapy modalities
    • Observe and report patient progress and/or changes in patient status to Physician and appropriate multi-disciplinary team members
    • Other duties as assigned
  • Job Requirements:
    • Master’s Degree from an accredited Occupational Therapy school
    • Good organizational skills
    • Good written skills
    • Valid New York State Occupational Therapy License

    Physical Therapist

    • Duties and Responsibilities:
      • Provide preventive services allowing individuals to stay in their homes and out of nursing home care
      • Identify and meet patients’ goals and needs
      • Offer cost-effective treatments that help improve clients’ motion and mobility
      • Reduce the need for medications and provide alternatives to surgery
      • Develop care plans using a variety of treatment techniques
      • Create fitness- and wellness-oriented programs tailored to patients’ specific needs
      • Provide quality, personalized and evidence-based care, and proven interventions
      • Motivate patients during treatment to help them function optimally
      • Promote clients’ healthy lifestyle by improving strength, flexibility, balance, and coordination
      • Consult and practice with other health professionals
      • Evaluate effects and monitor and communicate progress
      • Document patient care services
    • Job Requirements:
      • Valid NY State PT License
      • Master’s degree from an accredited physical therapy school
      • CPR Certification (Preferred)
      • Good organization skills
      • Good written skills

      Speech Language Pathologist

      • Duties and Responsibilities:
        • Absorbs responsibility for the screening, evaluation, and treatment of facility patients/residents needing Speech-Language Pathology services
        • Implements treatment plan through direct treatment, education of treatment staff and supervision of Speech-Language Pathology staff
        • Maintains accurate billing information in accordance with facility procedures on a daily basis
        • When appropriate, acts as a clinical instructor to SLP students who are engaged in their clinical residencies
      • Job Requirements:
        • Bachelor and/or Master’s degree in Speech and Language Pathology
        • Current and valid NY state license
        • Excellent oral and written communication skills
        • Computer literate
